slope of L = -1/5  and it passes through the point (-1,-5)
Point-slope form is  
y -(-5) = -1/5(x - (-1))

y + 5 = -1/5(x + 1)   <----- Point slope form


y = -1/5x - 1/5 - 5
y = -1/5x - 5 1/5 <------- Slope intercept form

To find the slope of a line perpendicular to another, you take the opposite reciprocal of it, or in your case, y=-1/5x. There also is a y-intercept which is -5 1/5, making the final equation y=-1/5x - 5 1/5.
